Default Armor Differences

Is there any armor out there that has the same NAME but a different LOOK?

IE: collector's "Monk Raiment" vs armorer "Monk Raiment"

Specifically, I'm curious at the differences between the various high end armors that have the same name.

Not as far as I know. Some of the collectors and basic crafted armors have the same names like your monk example. All the Fissure armors have the same look, but different names. Everything else has it's own name and look I'm pretty sure. I have noticed that my ranger's collector's Tamer's Mask looks different from the Ascalon crafted one I had previously.

Specifically, what high end armors are you referring to that have the same name?

What areas offer max level armor?

Droknar's, Marhan's Grotto, Granite Citadel, and FoW.

Yes.

Droknar's offers 1.5k, Citadel and Grotto offer 15k, and Fissure offers "I can't afford it". They all offer several sets and some have the same name, but each area has a different look. That link has pictures and names of all of them.

15k Tormentors looked the same as 1.5k Tormentors until about 8 (or so) hours ago.

Also Collector's Ranger masks have same name but look different (well, 2 atleast. Crafted Beast/Wilderness masks both have unique skins, but the collector Beast/Wilderness masks have the generic ranger mask skin).

Collectors in the desert and south shiverpeaks also offer max armor, but without any extra bonuses.
